Additional Fees: 

 

There is an additional $10 dollar fee for completion of forms when not provided at the time of their routine physical.  Including:

  • Day Care

  • School form

  • Sports Forms

  • Work Permits

  • Life Insurance

  • Family Medical Leave (FMLA) papers

If you have a private health insurance plan - you should always contact them to verify we are participating with them.
