FEES FOR COMPLETING FORMS

Forms are free of charge if presented at the time of your child's scheduled well-checkup.

If brought in at any time other than at a well-checkup, the following fees apply.

Standard Forms $15.00 (per child)

Driver's License

Sports

School/College

Day Care

FMLA $25.00

This applies to any form generated by an outside source or agency requiring a physician's signature.

Fees are collected at the time the form is dropped off.

For your convenience, every effort is made to complete the forms in 24 to 48 hours.
